I thought the the words were really about what to do after a Quiz. And based on this assumption, the ideas were good but they needed rearranging and some tender care.

Here is what I do: I proctor quizzes when the students are taking the exam in a lecture hall and are all in front of us using their laptops. My SOP is to look at the scores of the students on the info tab and make notes of all student comments (name and their question number) during the quiz(s). Sometimes I or the lead teacher will make a general announcement about a particular question. After the last student has finished the quiz, I perform an item analysis. 100% of the questions are multiple choice. I immediately look for questions that received the lowest percentage of correct answers. I look at the Disc columns to see how the best quiz takers did on the question. I write the question ID number and make a brief note. I repeat the process for the questions that student commented on during the test. Then I consult with content experts and the lead teacher using a form that has every question, it's answers and the answer's location in the presented material. We go over those marked questions and they may direct me to edit the answer scores, the question content, and/or mark the question heading so it is not used again. After following their directions, I regrade the quiz. This system has evolved over 4 cycles and roughly 150 quizzes in a certified learning environment.
