Overview
This form is used to contact the Privacy officer to make a Data Request under GDPR regulations.
Type of request
This specifies which type of request the user is making. When using this form two types of request can be made:
- Export all of my personal data
- Delete all of my personal data
Comments
This field allows the user to provide an explanation or reason for their request. It is optional.
