After setting a new Feedback you design it by &#039;&#039;&#039;adding questions&#039;&#039;&#039;. Select the type of question from the drop down list in the top left corner then click the &#039;&#039;Add question to activity&#039;&#039; button.&lt;br /&gt;

This will give you a series of options with a checkbox next to each one. The respondent can check as many as they want and there is currently no way to specify a limit to how many they can choose which is lower than the total.&lt;br /&gt;

It is possible to create a Public questionnaire which can be placed in several simultaneous courses, yet the responses are not visible to the teachers of those courses. In fact, the responses are only accessible in the original public questionnaire. A good application of this setup is for course evaluations which you want students to take. In such an example, you don&#039;t want your instructors to view or alter those responses. &lt;br /&gt;
&lt;br /&gt;
Here&#039;s how you do it:&lt;br /&gt;
&amp;lt;ol&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;Create a course that will simply be a shell for any public questionnaires. Give it whatever title you like. You can hide the course entirely. &amp;lt;/li&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;Create your questionnaire in this course. Be sure to select &#039;&#039;&#039;Public&#039;&#039;&#039; for the questionnaire type.&amp;lt;/li&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;Build all of the questionnaire questions in this public questionnaire.&amp;lt;/li&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;Navigate to the course where you want to place the questionnaire for respondents.&amp;lt;/li&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;Select the option to add a questionnaire. Complete the creation screen. For Type, select Respond once. For Respondent Eligibility, select Students only. &#039;&#039;&#039;For Questionnaire type, select Private.&#039;&#039;&#039; Then select the public questionnaire you created in the other course.&amp;lt;/li&amp;gt;&lt;br /&gt;
&amp;lt;li&amp;gt;After you save your changes, the same screen will appear. Note that the questionnaire type now says &amp;quot;Public,&amp;quot; and the selected public questionnaire is shown below.&amp;lt;/li&amp;gt;&lt;br /&gt;

:A simple YES/NO question. The Length and Precision parameters are not used so you can leave them as zero. Type your question in the text area. You do not need to supply possible answers for this field.&lt;br /&gt;

;Text&lt;br /&gt;
:Allows limited character text entry via a single line text area. Use length to define the visible width of the text box and Precision to determine the maximum number of characters allowed. If Precision is greater than Length then the text will extend within the box. Type your question or information in the Text area. You do not need to supply possible answers for this field.&lt;br /&gt;
&lt;br /&gt;
;Essay&lt;br /&gt;
:Allows essay style responses via a multi line text area, with no character limit. Length and Precision define the size of the available text area, although this will scroll. Length determines the number of columns (character width) and Precision the number of rows (lines). This can involve trial and error, although Length: 90, Precision: 7, should be a good general starting estimate. Type your question or information in the Text area. You do not need to supply possible answers for this field.&lt;br /&gt;

;Radio Buttons&lt;br /&gt;
:Radio buttons provide the user with a choice of options from which they can pick only one. This field ignores the Length and Precision parameters so you can leave them. Type your question or information in the Text area and the list of available options in the boxes underneath. You can add more possibilities using the &#039;&#039;Add another answer line&#039;&#039; button and add an other text field using &#039;&#039;&#039;!other&#039;&#039;&#039; as a possible answer. &lt;br /&gt;

:Check boxes allow the user to select multiple answers from a list of options. Length determines the minimum number of answers require and Precision the maximum number of selections allowed (these may not function fully as implemented). Ever wonder why you come home from a big party exhausted?  or why you have to make connections in your mind to understand new material?  I&#039;m going to send you out on a search for answers.  It won&#039;t take long...15-20 minutes and you will use your results to answer the questions in the questionnaire.  Then I will analyze your answers for trends and commonalities among us and report back to you.&lt;br /&gt;
