The Dataform module is an enhanced version of the standard Database activity module.  
The Dataform is a major enhancement of the Database activity module. Like the Database module, the Dataform allows for constructing an entry form from various field types in a flexible layout. Users can then add information records to a collection, which can be viewed in various ways and searched for particular items.
The module allows a course manager to define the structure and look of a content form, which the course participants can then fill out and submit. The form may include a variety of content entities such as images, files, URLs, numbers and text amongst other things. Submitted forms - entries - may be viewed in various ways.
