I was researching outcomes and got here.  This page needs some tender care for 2.x.  It is all over the place talking about gradebook, categories, and APIs. I was trying to redefine the menu path buried in "Activity based grade items" and felt helpless and lost in my Settings > Grade administration menu system when I clicked on grades.  Only one of many points.  I suspect that all we need is a simple definition and I do not think mine is correct. For example I wanted to see an Outcome report and this page says a grade item can be an  outcome or a summary of grades.  Thus a grade item is indeed a measurement of something.  It shows up as a column in the grade report according to its words.  This page explains too much.  I do use grades in quizzes on a regular basis. Possible improvements as of Dec 2010

'grading individual activities'
Grade_items - OU, Aug 10, 1.9 VI, good breakdown (Activity, Outcome, Manual) but need to clear up & offer examples (!) 
Grade_calculations - O, Dec 10, averages, sums, weightings ... link to example(s) would be good
Edit_grade_calculation re-directed to Grade_calculations but not sure if that's where I'd go for editing...
