Question set-up

I do not understand the must use 3 questions and answers instruction. Are we trying to say: "Each question must have 3 answers" ?

What is the purpose for providing an blank question with an answer? Can you give a brief example? I am being thick headed and lazy. I am more familiar with a Lesson question than a Quiz or Question bank question format.
